Position Summary:
We are seeking a dynamic and compassionate Nurse Practitioner (NP) to provide comprehensive primary care services to a diverse patient population, including adult and geriatric patients. The NP will be responsible for conducting patient assessments, managing acute and chronic conditions, prescribing medications, and delivering preventive care services. This role emphasizes collaboration with multidisciplinary teams and a strong focus on improving patient outcomes through education and evidence-based care.
Key Responsibilities:
• Perform comprehensive patient assessments including medical history, physical examinations, and diagnostic evaluations
• Diagnose and manage acute and chronic medical conditions
• Prescribe medications and develop individualized treatment plans
• Provide preventive care services and health screenings
• Educate patients on disease management, medications, and healthy lifestyle practices
• Collaborate with physicians and interdisciplinary healthcare teams to ensure coordinated care
• Support chronic disease and pain management initiatives
• Maintain accurate and timely documentation in the electronic medical record system
• Promote patient safety, quality care, and adherence to clinical standards
• Participate in team-based care and continuous quality improvement initiatives
Required Qualifications:
• Active, unrestricted Nurse Practitioner (NP) license in the State of Michigan
• Master’s degree in Nursing (MSN) from an accredited program
• Minimum 2 years of experience in patient assessment, chronic disease management, and primary care
• Strong experience in patient education, medication management, and clinical collaboration
• Excellent communication, clinical decision-making, and interpersonal skills
• Ability to work effectively in a multidisciplinary team environment
Preferred Qualifications:
• Experience with medical software and electronic medical record (EMR) systems
• Knowledge of advanced life support practices
• Experience in emergency medicine or acute care settings
• Familiarity with disease prevention and health promotion strategies
Pay Rate
$46.00 - $46.00/hour
