Licensed Vocational Nurse (LVN) – Skilled Nursing Facility (SNF)
We are seeking a compassionate and dependable LVN to provide quality nursing care to residents in a skilled nursing facility. The LVN will administer medications and treatments, monitor patient conditions, support daily care needs, and collaborate with the healthcare team.
Responsibilities:
- Provide direct nursing care according to individualized care plans.
- Administer medications and treatments as ordered.
- Monitor vital signs and changes in residents’ conditions.
- Provide wound care, dressing changes, catheter care, and other nursing treatments.
- Assist with hygiene, mobility, ADLs, and comfort needs.
- Document assessments, treatments, and patient care accurately.
- Report changes in condition to the RN or healthcare provider.
- Educate residents and families on care plans and health needs.
- Follow infection prevention, safety, and patient privacy standards.
- Collaborate with nurses, physicians, therapists, and other healthcare professionals.
Qualifications:
- Current LVN/LPN license.
- Graduate of an accredited practical/vocational nursing program.
- SNF, skilled nursing, or long-term care experience preferred.
- Knowledge of medication administration, wound care, and patient safety.
- Strong clinical judgment, communication, and organizational skills.
- Ability to manage multiple resident needs in a fast-paced environment.
- Compassionate, dependable, professional, and patient-focused.
Pay Rate
$52.85 - $57.72/hour
