LPN Case Manager – Job Description
- Provide care coordination and case management support to adult and geriatric patients as part of a multidisciplinary Transitions of Care team.
- Work under the direction of an RN Care Manager to implement care plans, monitor patient progress, and help reduce hospital readmissions.
- Conduct telephonic and remote follow-ups, including appointment scheduling, screening calls, triaging barriers, medication adherence, biometric monitoring, and patient education.
- Perform home, hospital, and provider-office visits as needed to observe, educate, and support patients; reliable transportation is required, but patient transportation is not required.
- Connect patients with community resources and support services, participate in virtual care-team rounds, and maintain frequent communication with patients and healthcare providers.
- Document patient interactions, assessments, and care coordination activities accurately in the EHR.
- Manage a rotating caseload of approximately 25–40 patients per month, reassessing goals during the 30-day program and transitioning or closing cases as appropriate.
- This is not a hands-on nursing position; strong communication, documentation, organization, independence, and knowledge of community resources are essential.
Pay Rate
$41.59 - $46.47/hour
