LVN / LPN

Position Summary:

We are seeking a compassionate and dedicated Licensed Vocational Nurse / Licensed Practical Nurse (LVN/LPN) to provide high-quality nursing care under the supervision of Registered Nurses and physicians. The LVN/LPN will assist with patient assessments, administer medications and treatments, monitor patient conditions, and collaborate with the healthcare team to ensure safe, effective, and patient-centered care.

Key Responsibilities:

• Provide direct patient care in accordance with physician orders, nursing care plans, and facility policies
• Monitor patient conditions, obtain vital signs, and report changes to the supervising Registered Nurse or healthcare provider
• Administer prescribed medications, injections, IV therapy (where permitted by state regulations), and treatments within the scope of practice
• Perform wound care, dressing changes, catheter care, specimen collection, and other clinical procedures
• Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, mobility, and nutrition
• Prepare patients for examinations, treatments, and diagnostic procedures
• Educate patients and families on medications, treatments, health maintenance, and discharge instructions
• Maintain accurate and timely documentation in Electronic Medical Records (EMR)
• Collaborate with physicians, Registered Nurses, therapists, and other healthcare professionals to coordinate patient care
• Assist with admissions, transfers, discharges, and continuity of care
• Ensure compliance with infection prevention protocols, HIPAA regulations, patient safety standards, and facility policies
• Promote a compassionate, respectful, and patient-centered care environment

Required Qualifications:

• Active, unrestricted Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license in the state of practice
• Graduate of an accredited Practical/Vocational Nursing program
• Current BLS/CPR certification required
• Minimum 1 year of recent LVN/LPN experience preferred
• Strong knowledge of nursing principles, medication administration, and patient care practices
• Excellent communication, organizational, interpersonal, and critical-thinking skills
• Ability to work effectively in a fast-paced healthcare environment
• Experience with Electronic Medical Record (EMR) systems preferred
