Position Summary:
We are seeking a detail-oriented Licensed Vocational Nurse (LVN/LPN) to serve as a Clinical Note Review Specialist. In this role, the LVN will review AI-flagged clinical documentation, validate compliance with regulatory and internal standards, identify documentation gaps, and support defensible billing practices. This position plays a critical role in ensuring documentation accuracy, compliance integrity, and audit readiness.
Key Responsibilities:
• Review AI-flagged clinical documentation for compliance with applicable payer and regulatory guidelines
• Validate documentation for completeness, accuracy, and clinical defensibility
• Identify documentation gaps, errors, or inconsistencies and escalate findings as appropriate
• Provide structured feedback to internal teams to support documentation improvement
• Document review findings accurately within compliance and audit systems
• Maintain clear audit trails and support case approval workflows
• Stay current on regulatory and compliance updates impacting clinical documentation
• Collaborate with internal teams to support quality and compliance objectives
Required Qualifications:
• Active, unrestricted LVN or LPN license
• Experience in clinical documentation review, utilization review, compliance, or chart auditing preferred
• Knowledge of wound care documentation, skin grafts, or advanced wound products preferred
• Strong attention to detail and ability to interpret clinical documentation accurately
• Familiarity with EMR systems and documentation standards
• Excellent written communication and analytical skills
• Ability to work independently in a remote environment
• Commitment to accuracy, compliance, and quality clinical documentation
Pay Rate
$33.93 - $38.81/hour
