LTC LPN
Position Summary
We are seeking a Long-Term Care (LTC) Licensed Practical Nurse (LPN) to provide safe and compassionate nursing care to residents with ongoing medical and daily care needs. The LPN will administer medications and treatments, monitor resident conditions, and collaborate with the healthcare team.
Responsibilities
Provide direct nursing care to residents according to care plans.
Administer medications, treatments, and other prescribed therapies.
Monitor vital signs, symptoms, and changes in resident condition.
Assist with wound care and other nursing procedures within scope of practice.
Support residents with activities of daily living and comfort needs.
Communicate changes in condition to the RN or healthcare provider.
Maintain accurate and timely clinical documentation.
Assist with care planning and coordination of resident services.
Follow infection control, safety, and long-term care standards.
Collaborate with RNs, physicians, CNAs, and other healthcare professionals.
Qualifications
Active LPN license in the applicable state.
Graduate of an accredited practical nursing program.
Previous long-term care, skilled nursing, or rehabilitation experience preferred.
Strong clinical assessment, communication, and organizational skills.
Ability to provide compassionate, patient-centered care.
Knowledge of medication administration, infection control, and nursing procedures.
Pay Rate
$45.22 - $50.10/hour
