LTC LPN
Position Summary
We are seeking a Long-Term Care Licensed Practical Nurse (LPN) to provide safe and compassionate nursing care to residents with ongoing medical and healthcare needs. The LPN will administer medications and treatments, monitor residents, assist with daily care, and support the healthcare team.
Responsibilities
- Provide direct nursing care under the direction of an RN or healthcare provider.
- Monitor vital signs, symptoms, and changes in resident condition.
- Administer medications and treatments as prescribed.
- Provide wound care and other nursing procedures within scope of practice.
- Assist with chronic disease management and ongoing care needs.
- Report changes in resident condition to the RN or healthcare provider.
- Assist residents with daily care and comfort needs.
- Educate residents and families regarding care and treatment.
- Maintain accurate and timely nursing documentation.
- Follow infection control, safety, and long-term care standards.
Qualifications
- Active LPN license as required by the state.
- Graduate of an accredited practical nursing program.
- Previous long-term care, skilled nursing, or geriatric experience preferred.
- Strong clinical, communication, and organizational skills.
- Ability to provide compassionate, resident-centered care.
- Ability to work effectively as part of a healthcare team.
Pay Rate
$45.22 - $50.10/hour
