Licensed Practical Nurse (LPN) – Long-Term Care (LTC)
Position Summary
Provide safe and compassionate nursing care to residents in a long-term care setting. Monitor resident conditions, administer medications and treatments, and support ongoing health, comfort, and quality of life.
Responsibilities
- Provide direct nursing care and monitor resident conditions.
- Administer medications and treatments as prescribed.
- Monitor vital signs, symptoms, and changes in resident status.
- Assist with wound care, catheter care, and other nursing procedures.
- Support residents with activities of daily living as needed.
- Develop and follow individualized care plans.
- Communicate changes in resident condition to the healthcare team.
- Educate residents and families on medications and care needs.
- Maintain accurate and timely clinical documentation.
- Follow infection-control, safety, and long-term care standards.
Qualifications
- Graduate of an accredited Practical Nursing program.
- Current and active LPN/LVN license in the applicable state.
- Previous long-term care or skilled nursing experience preferred.
- BLS certification required.
- Strong clinical, communication, organizational, and patient-care skills.
- Ability to work effectively as part of a multidisciplinary healthcare team.
- Full-time availability required.
Pay Rate
$37.60 - $42.48/hour
