Job Description – LPN
Provides direct patient care utilizing the nursing process under the direction and supervision of the RN, including rooming patients for in-person clinic visits and telemedicine appointments, obtaining relevant patient information, and assisting with patient care needs.
Maintains, reviews, and updates patient medical records, clinical notes, physician orders, and other healthcare documentation in accordance with established documentation standards and organizational policies.
Processes and collaborates on the management of electronic faxes, including appropriate review and integration into Epic to support timely and accurate patient care.
Reviews risk stratification forms for completeness and identifies information requiring follow-up, escalating concerns to the RN or appropriate provider as needed.
Coordinates gastroenterology procedures by contacting patients to confirm scheduled procedures and providing education regarding colonoscopy preparation and discharge instructions to promote patient understanding, safety, and adherence.
Supports patient communication and care coordination by reviewing patient portal messages, addressing appropriate inquiries within scope of practice, and escalating clinical concerns to the RN or provider as indicated.
Collaborates with nurses, providers, and interdisciplinary team members to facilitate efficient clinic operations, timely patient care, and continuity of care.
Participates in unit teams, work groups, committees, and other departmental activities as assigned.
Participates in quality and performance improvement initiatives by promoting accurate documentation, effective patient communication, timely procedure coordination, and efficient clinical workflows.
Performs other duties as assigned in support of the gastroenterology clinic, nursing team, and overall delivery of patient care.
Pay Rate
$38.32 - $43.20/hour
