Licensed Practical Nurse (LPN) – Post-Acute Long-Term Care (LTC)
We are seeking a compassionate and dependable LPN to provide nursing care to patients in a post-acute and long-term care setting. The LPN will provide medications and treatments, monitor patient conditions, support daily care needs, and collaborate with the healthcare team.
Responsibilities:
- Provide direct nursing care according to patient care plans.
- Administer medications and treatments as ordered.
- Monitor vital signs, patient conditions, and changes in health status.
- Provide wound care, dressing changes, catheter care, and other nursing treatments.
- Assist with ADLs, mobility, hygiene, and comfort needs.
- Document patient care, assessments, and changes accurately.
- Communicate patient concerns and changes to the RN or healthcare provider.
- Educate patients and families on care plans and health needs.
- Follow infection prevention, safety, and patient privacy standards.
- Collaborate with nurses, physicians, therapists, and other care team members.
Qualifications:
- Current LPN/LVN license.
- Graduate of an accredited practical nursing program.
- Post-acute, skilled nursing, or long-term care experience preferred.
- Knowledge of medication administration, wound care, and patient safety.
- Strong clinical judgment, communication, and organizational skills.
- Ability to manage multiple patient needs in a fast-paced environment.
- Compassionate, dependable, professional, and patient-focused.
Pay Rate
$45.22 - $50.10/hour
