Position Summary:
We are seeking a compassionate and dependable LPN to provide nursing care to residents in a long-term care setting. The LPN will administer medications and treatments, monitor residents' conditions, assist with daily care needs, and work closely with RNs, providers, residents, and families.
Key Responsibilities:
- Provide direct nursing care to residents according to individualized care plans.
- Monitor residents' health status and report changes promptly to the RN or healthcare provider.
- Administer medications and treatments within scope of practice.
- Perform wound care, dressing changes, catheter care, and other nursing procedures.
- Monitor and document vital signs, intake/output, and other clinical information.
- Assist residents with activities of daily living, mobility, transfers, and personal care as needed.
- Support residents with chronic disease management and ongoing healthcare needs.
- Collect specimens and assist with diagnostic procedures as directed.
- Communicate with physicians, RNs, CNAs, families, and other members of the care team.
- Participate in care planning, resident assessments, and interdisciplinary meetings.
- Educate residents and families regarding medications, treatments, and health conditions.
- Maintain accurate and timely documentation in the EMR.
- Follow infection prevention, HIPAA, patient safety, and LTC policies.
- Promote resident dignity, independence, comfort, and quality of life.
Required Qualifications:
- Graduate of an accredited Practical Nursing program.
- Active, unrestricted LPN/LVN license in the state of practice.
- Current BLS certification required.
- Minimum 1 year of recent long-term care, skilled nursing, or post-acute experience preferred.
- Strong knowledge of medication administration, wound care, patient assessment, and nursing procedures.
- Excellent communication, organization, clinical judgment, and time-management skills.
- Ability to manage multiple residents and changing priorities.
- Experience with EMR systems preferred.
Preferred Qualifications:
- Previous long-term care, skilled nursing, rehabilitation, or post-acute experience.
- Experience caring for geriatric and medically complex residents.
- Wound care and IV therapy experience preferred.
- Knowledge of care planning and chronic disease management.
- Strong resident and family communication skills.
- Commitment to compassionate, person-centered care.
Pay Rate
$37.60 - $42.48/hour
