Position Summary:
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide quality nursing care to residents in a Long-Term Care (LTC) setting under the supervision of a Registered Nurse or physician. The LPN will assist with assessments, medication administration, treatments, and resident care while promoting safety, comfort, dignity, and quality of life.
Key Responsibilities:
- Provide direct nursing care to long-term care residents under the supervision of a Registered Nurse or physician.
- Monitor and document resident vital signs, symptoms, and changes in condition.
- Administer prescribed medications, injections, IV therapy (where permitted), and treatments.
- Perform wound care, dressing changes, catheter care, specimen collection, and other nursing procedures within scope of practice.
- Assist with resident admissions, transfers, and discharges.
- Support residents with activities of daily living (ADLs) while promoting comfort and independence.
- Educate residents and families regarding medications, treatments, and care plans.
- Maintain accurate documentation in the Electronic Medical Record (EMR).
- Collaborate with physicians, registered nurses, therapists, social workers, and other healthcare professionals to coordinate resident care.
- Supervise Certified Nursing Assistants (CNAs) as assigned.
- Ensure compliance with infection prevention protocols, HIPAA regulations, patient safety standards, and facility policies.
- Participate in quality improvement initiatives and continuing education activities.
Required Qualifications:
- Graduate of an accredited Practical Nursing program.
- Active, unrestricted Licensed Practical Nurse (LPN) license in the state of practice.
- Current BLS certification required.
- Minimum 1 year of recent Long-Term Care (LTC), Skilled Nursing Facility (SNF), rehabilitation, or geriatric nursing experience preferred.
- Strong knowledge of medication administration, wound care, chronic disease management, and geriatric nursing.
- Excellent communication, organizational, critical-thinking, and interpersonal skills.
- Experience with Electronic Medical Record (EMR) systems preferred.
Pay Rate
$49.03 - $53.91/hour
