Job Description

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide high-quality nursing care in a Long-Term Care/Skilled Nursing setting. The LPN will be responsible for administering medications and treatments, monitoring patient conditions, documenting care accurately, and collaborating with physicians, Registered Nurses, and the interdisciplinary healthcare team to ensure the best possible patient outcomes.

Key Responsibilities

Administer medications and treatments as prescribed.
Monitor and document patients' vital signs and health status.
Assist patients with activities of daily living (ADLs).
Perform basic wound care and other nursing procedures.
Maintain accurate patient records and EMR documentation.
Observe and report changes in patient conditions.
Follow infection control and safety protocols.
Collaborate with RNs, physicians, and other healthcare professionals to deliver quality patient care.
Educate patients and families on treatment plans and health maintenance.
Ensure compliance with facility policies and nursing standards.

Pay Rate
$17.00 - $17.00/hour
