Position Summary:
We are seeking a compassionate and dependable Licensed Practical Nurse to provide direct patient care under the supervision of an RN or healthcare provider. The LPN will administer medications and treatments, monitor patient conditions, assist with daily care needs, and support the healthcare team in delivering safe, high-quality patient care.
Key Responsibilities:
- Perform basic patient assessments and monitor changes in condition.
- Administer medications and treatments within scope of practice.
- Provide wound care, dressing changes, catheter care, and other nursing procedures.
- Monitor and document vital signs, intake/output, and patient information.
- Assist patients with activities of daily living, mobility, and personal care.
- Collect specimens and assist with diagnostic procedures as directed.
- Implement assigned portions of individualized care plans.
- Educate patients and families regarding medications, treatments, and health management.
- Report changes in patient condition promptly to the RN or healthcare provider.
- Maintain accurate and timely documentation in the EMR.
- Collaborate with RNs, physicians, therapists, aides, and other healthcare professionals.
- Follow infection prevention, HIPAA, patient safety, and organizational policies.
- Provide compassionate, respectful, and patient-centered care.
Required Qualifications:
- Graduate of an accredited Practical Nursing program.
- Active, unrestricted LPN/LVN license in the state of practice.
- Current BLS certification required.
- Minimum 1 year of recent LPN/LVN experience preferred.
- Strong knowledge of medication administration, patient care, wound care, and nursing procedures.
- Excellent communication, clinical judgment, organization, and time-management skills.
- Ability to work independently and effectively within a multidisciplinary team.
- Experience with EMR systems preferred.
Preferred Qualifications:
- Experience in acute care, long-term care, skilled nursing, rehabilitation, outpatient, or home health settings.
- Experience caring for geriatric and medically complex patients.
- Strong patient and family education skills.
- Experience with electronic documentation and medication administration systems.
- Commitment to safe, compassionate, patient-centered care.
Pay Rate
$25.89 - $30.77/hour
