Position Summary:

We are seeking a compassionate and dependable Licensed Practical Nurse to provide direct nursing care to patients under the direction of an RN or healthcare provider. The LPN will monitor patient conditions, administer medications and treatments, provide routine nursing care, and support patients and families.

Key Responsibilities:

  • Perform patient assessments and monitor vital signs and changes in condition.
  • Administer medications and treatments within scope of practice.
  • Provide wound care, dressing changes, catheter care, and other nursing procedures.
  • Assist patients with activities of daily living, mobility, and personal care.
  • Collect specimens and monitor laboratory and diagnostic results as required.
  • Report changes in patient condition promptly to the RN or healthcare provider.
  • Assist with care planning and implementation of individualized treatment plans.
  • Educate patients and families regarding medications, treatments, and health management.
  • Maintain accurate and timely documentation in the EMR.
  • Collaborate with RNs, physicians, therapists, aides, and other healthcare professionals.
  • Follow infection prevention, HIPAA, patient safety, and organizational policies.
  • Provide compassionate, patient-centered care in a professional manner.

Required Qualifications:

  • Graduate of an accredited Practical Nursing program.
  • Active, unrestricted LPN/LVN license in the state of practice.
  • Current BLS certification required.
  • Minimum 1 year of recent LPN/LVN experience preferred.
  • Strong knowledge of medication administration, patient care, wound care, and basic nursing procedures.
  • Excellent communication, clinical judgment, organization, and time-management skills.
  • Ability to work independently and effectively within a multidisciplinary team.
  • Experience with EMR systems preferred.

Preferred Qualifications:

  • Previous experience in acute care, long-term care, skilled nursing, rehabilitation, outpatient, or home health settings.
  • Experience caring for medically complex and diverse patient populations.
  • Strong patient and family education skills.
  • Experience with electronic documentation and medication administration systems.
  • Commitment to safe, compassionate, and high-quality patient care.

Pay Rate
$25.51 - $30.39/hour
