Position Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide quality nursing care under the supervision of a Registered Nurse or physician. The LPN will assist with patient assessments, administer medications and treatments, monitor patient conditions, and support the healthcare team in delivering safe, efficient, and patient-centered care.
Key Responsibilities:
- Provide direct patient care according to individualized care plans and physician orders.
- Administer medications, injections, IV therapy (where permitted by state regulations), and prescribed treatments.
- Monitor and document patient conditions, vital signs, and report changes to the supervising nurse or provider.
- Perform wound care, dressing changes, catheter care, specimen collection, and other nursing procedures.
- Assist with patient admissions, transfers, and discharge planning.
- Educate patients and families on medications, treatments, and disease management.
- Maintain accurate and timely documentation in the Electronic Medical Record (EMR).
- Collaborate with physicians, Registered Nurses, therapists, and other healthcare professionals to coordinate patient care.
- Ensure compliance with infection prevention protocols, HIPAA regulations, patient safety standards, and facility policies.
- Participate in quality improvement initiatives and continuing education activities.
Required Qualifications:
- Graduate of an accredited Practical Nursing program.
- Active, unrestricted Licensed Practical Nurse (LPN) license in the state of practice.
- Current BLS certification required.
- Minimum 1 year of recent LPN experience preferred.
- Strong knowledge of medication administration, patient care, and infection prevention.
- Excellent communication, organizational, and interpersonal skills.
- Experience with Electronic Medical Record (EMR) systems preferred.
Preferred Qualifications:
- Experience in acute care, long-term care, skilled nursing facilities (SNF), rehabilitation, outpatient clinics, or home health settings.
- IV Therapy certification where permitted by state regulations.
- Previous travel or contract nursing experience preferred.
- Strong teamwork, adaptability, and commitment to delivering compassionate, patient-centered care.
Pay Rate
$27.22 - $32.10/hour
