Position Summary:
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide high-quality nursing care under the supervision of a Registered Nurse or physician. The LPN will assist with patient assessments, administer medications, perform treatments, monitor patient conditions, and support the healthcare team in delivering safe, patient-centered care.
Key Responsibilities:
- Provide direct patient care in accordance with individualized care plans and physician orders.
- Administer medications, injections, IV therapy (where permitted), and prescribed treatments.
- Monitor patient conditions, vital signs, and report changes to the supervising Registered Nurse or provider.
- Assist with wound care, catheter care, dressing changes, specimen collection, and other nursing procedures.
- Support patient admissions, transfers, and discharges.
- Educate patients and families on medications, treatments, disease management, and discharge instructions.
- Maintain accurate and timely documentation in the Electronic Medical Record (EMR).
- Collaborate with physicians, Registered Nurses, therapists, and other healthcare professionals to coordinate patient care.
- Ensure compliance with infection prevention protocols, HIPAA regulations, patient safety standards, and facility policies.
- Participate in quality improvement initiatives and continuing education activities.
Required Qualifications:
- Graduate of an accredited Practical Nursing program.
- Active, unrestricted Licensed Practical Nurse (LPN) license in the state of practice.
- Current BLS certification required.
- Minimum 1 year of recent LPN experience preferred.
- Knowledge of medication administration, patient assessment, basic nursing procedures, and infection prevention practices.
- Excellent communication, organizational, and interpersonal skills.
- Experience with Electronic Medical Record (EMR) systems preferred.
Pay Rate
$26.46 - $31.34/hour
