Position Summary:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide safe, effective, and patient-centered nursing care across various healthcare settings. The LPN will support patient care through assessments, medication administration, treatments, documentation, and collaboration with Registered Nurses, physicians, and interdisciplinary teams to promote positive patient outcomes.
Key Responsibilities:
• Provide direct nursing care to patients under the direction of Registered Nurses (RNs), physicians, and healthcare providers
• Perform patient assessments and monitor patient conditions, symptoms, and responses to treatment
• Administer medications, treatments, and therapies within the scope of practice and facility guidelines
• Monitor vital signs and report changes in patient condition to appropriate healthcare team members
• Assist with wound care, catheter care, respiratory treatments, and other nursing procedures as assigned
• Develop and implement care activities based on patient needs and established care plans
• Maintain accurate and timely documentation of patient care, interventions, and outcomes in electronic medical records (EMR)
• Educate patients and families regarding medications, treatments, and care instructions
• Assist patients with activities of daily living (ADLs), mobility, comfort, and safety needs
• Collaborate with RNs, physicians, therapists, and interdisciplinary healthcare teams
• Follow infection prevention practices, safety protocols, and regulatory standards
• Support continuity of care and promote a positive patient experience
• Maintain professionalism, confidentiality, and compassionate patient interactions
Required Qualifications:
• Active, unrestricted Licensed Practical Nurse (LPN/LVN) license in the state of practice
• Completion of an accredited Practical Nursing program
• Current BLS certification required
• Minimum 1–2 years of recent LPN experience preferred
• Strong knowledge of nursing principles, patient care procedures, and clinical documentation
• Excellent communication, teamwork, and organizational skills
• Ability to work effectively in a fast-paced healthcare environment
• Strong attention to detail and commitment to patient safety
• Experience with electronic medical record (EMR) systems preferred
Pay Rate
$25.89 - $30.77/hour
