Licensed Master Social Worker (LMSW)
We are seeking a compassionate and skilled Licensed Master Social Worker (LMSW) to provide social work services, counseling, care coordination, and support to patients and families.
Responsibilities:
- Assess patients' psychosocial needs and develop appropriate care plans.
- Provide counseling, emotional support, and crisis intervention.
- Coordinate referrals to healthcare and community resources.
- Assist with discharge planning and transitions of care.
- Advocate for patients and help address social and financial barriers.
- Collaborate with physicians, nurses, and interdisciplinary teams.
- Maintain accurate case notes and clinical documentation.
- Follow ethical, confidentiality, and organizational standards.
Qualifications:
- Master’s degree in Social Work (MSW) from an accredited program.
- Active LMSW or applicable state social work license.
- Strong knowledge of counseling, care coordination, and community resources.
- Excellent communication, assessment, and problem-solving skills.
- Previous healthcare, hospital, behavioral health, or case management experience preferred.
Pay Rate
$71.90 - $76.78/hour
