Licensed Clinical Medical Social Worker (MSW)
We are seeking a compassionate Licensed Clinical Medical Social Worker to provide psychosocial assessments, counseling, and care coordination for patients and families in a healthcare setting.
Responsibilities:
- Conduct psychosocial assessments and develop individualized care plans.
- Provide counseling, crisis intervention, and emotional support.
- Coordinate discharge planning and connect patients with community resources.
- Collaborate with physicians, nurses, and the interdisciplinary healthcare team.
- Advocate for patients and families to ensure appropriate care and support.
- Maintain accurate documentation and comply with regulatory standards.
Qualifications:
- Master's degree in Social Work (MSW) from an accredited program.
- Active and unrestricted Clinical Social Worker license (LCSW/LMSW or equivalent, as required by state regulations).
- Strong knowledge of case management, counseling, and community resources.
- Excellent communication, interpersonal, and problem-solving skills.
- Previous hospital, healthcare, or clinical social work experience preferred.
Pay Rate
$64.28 - $69.16/hour
