Inpatient Pharmacist (IP Pharmacist)
Provide comprehensive pharmacy services to hospitalized patients to ensure safe, effective, and appropriate medication therapy. The Inpatient Pharmacist will review medication orders, monitor drug therapy, collaborate with the healthcare team, and support optimal patient outcomes.
Responsibilities
- Review and verify medication orders for accuracy, safety, and appropriateness.
- Dispense medications according to physician orders and established procedures.
- Monitor patient medication therapy, allergies, interactions, and potential adverse effects.
- Provide recommendations regarding drug selection, dosing, and therapy adjustments.
- Participate in patient rounds and collaborate with physicians, nurses, and other healthcare professionals.
- Provide medication information and education to patients and clinical staff.
- Perform medication reconciliation and support discharge medication planning.
- Maintain accurate pharmacy records and documentation.
- Follow medication safety, infection control, and regulatory standards.
- Support quality improvement and medication-use initiatives.
Qualifications
- Doctor of Pharmacy (PharmD) or equivalent pharmacy degree.
- Current Pharmacist license in the applicable state.
- Previous inpatient or hospital pharmacy experience preferred.
- Strong knowledge of pharmacology, medication safety, and clinical pharmacy practices.
- Excellent clinical, analytical, and communication skills.
- Ability to work effectively in a fast-paced hospital environment.
Pay Rate
$75.71 - $80.59/hour
