Position Summary:
We are seeking a knowledgeable and detail-oriented Inpatient Pharmacist to provide safe and effective medication management for hospitalized patients. The Pharmacist will review and verify medication orders, provide clinical pharmacy services, collaborate with healthcare providers, and support medication safety throughout the hospital.
Key Responsibilities:
- Review, verify, and dispense medications according to physician orders and hospital protocols
- Evaluate medication orders for appropriate dosing, interactions, allergies, contraindications, and duplications
- Provide clinical pharmacy recommendations to physicians and other healthcare professionals
- Monitor medication therapy, therapeutic outcomes, and potential adverse drug reactions
- Prepare and oversee sterile and non-sterile medications as required
- Manage IV medications, controlled substances, high-alert medications, and specialty therapies
- Participate in medication reconciliation, antimicrobial stewardship, and clinical pharmacy initiatives
- Provide medication counseling and education to patients and healthcare staff when appropriate
- Maintain accurate medication records and documentation in the pharmacy system
- Ensure compliance with state and federal pharmacy regulations, USP standards, and hospital policies
- Support medication safety, quality improvement, and pharmacy operations
Required Qualifications:
- Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited program
- Active, unrestricted Pharmacist license in the state of practice
- Minimum 1 year of recent inpatient or hospital pharmacy experience preferred
- Strong knowledge of pharmacotherapy, medication safety, and pharmacy law
- Experience with pharmacy information systems and electronic medical records (EMR) preferred
- Excellent clinical, communication, organizational, and problem-solving skills
- Ability to work effectively in a fast-paced hospital environment
Pay Rate
$92.48 - $97.36/hour
