Position Summary:
We are seeking a knowledgeable and patient-focused Inpatient Pharmacist to provide comprehensive pharmaceutical care in a hospital setting. The Pharmacist will review medication orders, prepare and dispense medications, monitor drug therapy, provide clinical recommendations, and collaborate with physicians, nurses, and other healthcare professionals to ensure safe and effective medication management.
Key Responsibilities:
- Review and verify medication orders for accuracy, appropriateness, and potential drug interactions
- Prepare, compound, dispense, and oversee the safe distribution of medications in accordance with hospital policies and regulatory standards
- Monitor patient medication therapy and recommend dosage adjustments based on clinical status, laboratory values, and renal/hepatic function
- Participate in interdisciplinary rounds and provide evidence-based medication recommendations
- Counsel healthcare providers on medication selection, dosing, compatibility, and administration
- Ensure safe handling and documentation of controlled substances and high-alert medications
- Supervise and support Pharmacy Technicians in medication preparation and distribution
- Maintain accurate medication records and documentation in Electronic Medical Records (EMR) and pharmacy information systems
- Respond to medication-related emergencies and assist with code blue or rapid response situations as needed
- Ensure compliance with hospital policies, Joint Commission standards, state and federal regulations, and medication safety protocols
- Participate in antimicrobial stewardship, medication safety, and quality improvement initiatives
- Stay current with clinical guidelines, pharmacy best practices, and continuing education requirements
Required Qualifications:
- Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited program
- Active, unrestricted Pharmacist license in the state of practice
- Minimum 1 year of recent inpatient hospital pharmacy experience preferred
- Strong knowledge of pharmacotherapy, sterile compounding, medication safety, and hospital pharmacy operations
- Experience reviewing medication profiles, dosing adjustments, IV medications, and clinical interventions
- Excellent communication, critical-thinking, organizational, and problem-solving skills
- Experience with Electronic Medical Records (EMR), computerized physician order entry (CPOE), and pharmacy information systems
- Ability to work independently and collaboratively in a fast-paced hospital environment
