Hospice / Medical Social Worker
We are seeking a compassionate and skilled Hospice/Medical Social Worker to provide psychosocial support, counseling, and care coordination for patients and families facing serious or end-of-life illness.
Key Responsibilities:
- Assess patients’ and families’ psychosocial, emotional, and support needs.
- Provide counseling, emotional support, and crisis intervention.
- Assist patients and families with coping, grief, and end-of-life concerns.
- Coordinate community resources, referrals, and supportive services.
- Collaborate with nurses, physicians, and the interdisciplinary hospice team.
- Maintain accurate clinical documentation and care plans.
Qualifications:
- Master’s degree in Social Work (MSW) from an accredited program.
- Current LCSW/LICSW or applicable state social work license preferred/required.
- Previous hospice, palliative care, or medical social work experience preferred.
- Strong communication, counseling, crisis-management, and interpersonal skills.
Pay Rate
$48.27 - $53.15/hour
