Hospice RN
Provide compassionate, patient-centered nursing care to individuals with life-limiting illnesses while supporting comfort, dignity, and quality of life. The Hospice RN will assess patient needs, manage symptoms, coordinate care, and provide education and emotional support to patients and families.
Responsibilities
- Perform comprehensive patient assessments and develop individualized care plans.
- Manage pain, symptoms, medications, and comfort measures.
- Monitor patient condition and adjust care based on changing needs.
- Provide wound care and other skilled nursing interventions as appropriate.
- Educate patients and caregivers on medications, symptom management, and end-of-life care.
- Provide emotional support and compassionate guidance to patients and families.
- Coordinate care with physicians, hospice aides, social workers, chaplains, and other team members.
- Respond to patient and caregiver concerns and provide timely interventions.
- Maintain accurate and timely clinical documentation.
- Promote patient dignity, comfort, safety, and quality of life.
Qualifications
- Current Registered Nurse (RN) license.
- Graduate of an accredited nursing program.
- Previous hospice, palliative care, home health, or relevant nursing experience preferred.
- Strong assessment, symptom-management, and communication skills.
- Compassionate and patient-centered approach to care.
- Ability to work independently and effectively with patients and families.
Pay Rate
$64,480.00 - $93,350.00/hour
