Position Summary:
We are seeking a compassionate and dedicated Hospice Registered Nurse (RN) to provide skilled nursing care and emotional support to patients with terminal illnesses and their families. The Hospice RN will assess patient needs, develop individualized care plans, manage symptoms, coordinate interdisciplinary care, and promote comfort, dignity, and quality of life throughout the end-of-life journey.
Key Responsibilities:
- Perform comprehensive patient assessments and develop individualized hospice care plans
- Provide skilled nursing care focused on pain and symptom management
- Administer medications, IV therapies, and treatments as prescribed
- Monitor patient condition and adjust care plans in collaboration with the hospice team
- Educate patients and families on disease progression, medications, comfort measures, and end-of-life care
- Provide emotional support, counseling, and compassionate care to patients and caregivers
- Coordinate care with physicians, social workers, chaplains, therapists, and other interdisciplinary team members
- Respond to urgent patient needs and participate in on-call coverage as required
- Maintain accurate and timely documentation in Electronic Medical Records (EMR)
- Assist with admissions, recertifications, and discharge planning when appropriate
- Ensure compliance with hospice regulations, infection prevention protocols, HIPAA, and patient safety standards
- Advocate for patient comfort, dignity, and family-centered care
Required Qualifications:
- Active, unrestricted Registered Nurse (RN) license in the state of practice
- Graduate of an accredited Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) program
- Current BLS/CPR certification required
- Minimum 1 year of recent RN experience required
- Hospice, home health, palliative care, oncology, or medical-surgical experience preferred
- Strong knowledge of pain management, symptom control, end-of-life care, and hospice regulations
- Excellent communication, critical-thinking, organizational, and interpersonal skills
- Experience with Electronic Medical Record (EMR) systems preferred
- Valid driver's license and reliable transportation, if home visits are required
- Commitment to delivering compassionate, patient- and family-centered care
