Position Summary:
We are seeking a compassionate and dedicated Outpatient Hospice Registered Nurse (RN) to provide high-quality hospice care to patients in outpatient and home-based settings. The RN will assess patient needs, manage pain and symptoms, coordinate interdisciplinary care, educate patients and families, and promote comfort, dignity, and quality of life throughout the end-of-life journey.
Key Responsibilities:
- Perform comprehensive patient assessments and develop individualized hospice care plans
- Provide skilled nursing care focused on pain and symptom management
- Administer medications, treatments, and therapies as prescribed
- Monitor patient condition and adjust care plans in collaboration with the interdisciplinary team
- Educate patients and families on disease progression, medications, comfort measures, and end-of-life care
- Provide emotional support and compassionate care to patients and caregivers
- Coordinate care with physicians, social workers, chaplains, therapists, and other healthcare professionals
- Conduct outpatient and home visits as assigned while ensuring continuity of care
- Maintain accurate and timely documentation in Electronic Medical Records (EMR)
- Assist with admissions, recertifications, discharge planning, and care transitions
- Ensure compliance with hospice regulations, HIPAA, infection prevention, and patient safety standards
- Participate in on-call coverage and quality improvement initiatives as required
Required Qualifications:
- Active, unrestricted Registered Nurse (RN) license in the state of practice
- Graduate of an accredited Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) program
- Current BLS/CPR certification required
- Minimum 1 year of recent RN experience required
- Hospice, palliative care, home health, oncology, or medical-surgical nursing experience preferred
- Strong knowledge of pain management, symptom control, hospice regulations, and end-of-life care
- Excellent communication, critical-thinking, organizational, and interpersonal skills
- Experience with Electronic Medical Record (EMR) systems preferred
- Valid driver's license and reliable transportation for patient visits
- Ability to work independently and effectively manage a patient caseload
