Position Summary:

We are seeking a compassionate and dedicated Outpatient Hospice Registered Nurse (RN) to provide high-quality hospice care to patients in outpatient and home-based settings. The RN will assess patient needs, manage pain and symptoms, coordinate interdisciplinary care, educate patients and families, and promote comfort, dignity, and quality of life throughout the end-of-life journey.


Key Responsibilities:

  • Perform comprehensive patient assessments and develop individualized hospice care plans
  • Provide skilled nursing care focused on pain and symptom management
  • Administer medications, treatments, and therapies as prescribed
  • Monitor patient condition and adjust care plans in collaboration with the interdisciplinary team
  • Educate patients and families on disease progression, medications, comfort measures, and end-of-life care
  • Provide emotional support and compassionate care to patients and caregivers
  • Coordinate care with physicians, social workers, chaplains, therapists, and other healthcare professionals
  • Conduct outpatient and home visits as assigned while ensuring continuity of care
  • Maintain accurate and timely documentation in Electronic Medical Records (EMR)
  • Assist with admissions, recertifications, discharge planning, and care transitions
  • Ensure compliance with hospice regulations, HIPAA, infection prevention, and patient safety standards
  • Participate in on-call coverage and quality improvement initiatives as required

Required Qualifications:

  • Active, unrestricted Registered Nurse (RN) license in the state of practice
  • Graduate of an accredited Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) program
  • Current BLS/CPR certification required
  • Minimum 1 year of recent RN experience required
  • Hospice, palliative care, home health, oncology, or medical-surgical nursing experience preferred
  • Strong knowledge of pain management, symptom control, hospice regulations, and end-of-life care
  • Excellent communication, critical-thinking, organizational, and interpersonal skills
  • Experience with Electronic Medical Record (EMR) systems preferred
  • Valid driver's license and reliable transportation for patient visits
  • Ability to work independently and effectively manage a patient caseload
