Hospice Admission Visit Registered Nurse (RN)

Position Summary

Provide comprehensive nursing assessments and hospice admission services for patients and families receiving end-of-life care. Develop individualized care plans, provide education and support, and coordinate services to promote comfort and quality of life.

Responsibilities

  • Conduct hospice admission assessments and evaluate patient and family needs.
  • Review medical history, medications, symptoms, and current health status.
  • Develop individualized hospice care plans in collaboration with the interdisciplinary team.
  • Provide symptom management and comfort-focused nursing care.
  • Educate patients and caregivers on medications, treatments, safety, and hospice services.
  • Coordinate care with physicians, hospice staff, and other healthcare professionals.
  • Recognize changes in patient condition and communicate concerns promptly.
  • Provide emotional support to patients and families.
  • Complete accurate and timely admission and clinical documentation.
  • Follow hospice, patient safety, infection-control, and clinical standards.

Qualifications

  • Graduate of an accredited nursing program.
  • Current and active RN license in the applicable state.
  • Previous hospice, palliative care, home health, or community nursing experience preferred.
  • Strong assessment, communication, organizational, and patient-care skills.
  • Ability to work independently and provide compassionate end-of-life care.

Pay Rate
$56.66 - $61.53/hour
