Hospice Admission Visit Registered Nurse (RN)
Position Summary
Provide comprehensive nursing assessments and hospice admission services for patients and families receiving end-of-life care. Develop individualized care plans, provide education and support, and coordinate services to promote comfort and quality of life.
Responsibilities
- Conduct hospice admission assessments and evaluate patient and family needs.
- Review medical history, medications, symptoms, and current health status.
- Develop individualized hospice care plans in collaboration with the interdisciplinary team.
- Provide symptom management and comfort-focused nursing care.
- Educate patients and caregivers on medications, treatments, safety, and hospice services.
- Coordinate care with physicians, hospice staff, and other healthcare professionals.
- Recognize changes in patient condition and communicate concerns promptly.
- Provide emotional support to patients and families.
- Complete accurate and timely admission and clinical documentation.
- Follow hospice, patient safety, infection-control, and clinical standards.
Qualifications
- Graduate of an accredited nursing program.
- Current and active RN license in the applicable state.
- Previous hospice, palliative care, home health, or community nursing experience preferred.
- Strong assessment, communication, organizational, and patient-care skills.
- Ability to work independently and provide compassionate end-of-life care.
Pay Rate
$56.66 - $61.53/hour
