Home Health Registered Nurse (RN)
Position Summary
Provide skilled nursing care to patients in their homes, focusing on assessment, treatment, education, and care coordination. Support patients in achieving safe recovery and maintaining health and independence at home.
Responsibilities
- Perform comprehensive patient assessments and monitor health conditions.
- Develop and implement individualized home health care plans.
- Administer medications, treatments, IV therapy, and wound care as prescribed.
- Monitor vital signs, symptoms, and changes in patient condition.
- Educate patients and caregivers on medications, treatments, and disease management.
- Coordinate care with physicians, therapists, and other healthcare professionals.
- Maintain accurate and timely clinical documentation.
- Respond appropriately to changes in patient condition and communicate concerns.
- Follow infection-control, safety, and home health standards.
Qualifications
- Graduate of an accredited nursing program.
- Current and active RN license in the applicable state.
- Previous home health or community-based nursing experience preferred.
- Strong assessment, communication, organization, and patient-care skills.
- Ability to work independently and manage multiple patient visits effectively.
Pay Rate
$52.85 - $57.72/hour
