Registered Nurse – Home Health (Home Health RN)
We are seeking a compassionate and independent Registered Nurse to provide skilled nursing care to patients in their homes. The Home Health RN will assess patient needs, develop individualized care plans, deliver clinical care, and educate patients and caregivers to promote recovery, independence, and overall well-being.
Responsibilities:
• Conduct comprehensive patient assessments in the home setting.
• Develop and implement individualized care plans.
• Administer medications, wound care, IV therapies, and other prescribed treatments.
• Monitor patient progress and communicate changes to physicians and the healthcare team.
• Educate patients and caregivers on disease management, medications, and home care techniques.
• Coordinate care with interdisciplinary healthcare professionals to ensure continuity of care.
• Maintain accurate and timely documentation of patient visits and treatments.
Qualifications:
• Active and unrestricted Registered Nurse (RN) license.
• Graduation from an accredited nursing program.
• BLS certification required.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently and manage a flexible caseload.
• Previous home health, community health, or acute care nursing experience preferred.
Pay Rate
$68.09 - $72.97/hour
