Home Health Physical Therapist (PT)
We are seeking a skilled Home Health Physical Therapist (PT) to provide individualized rehabilitation services to patients in their homes and help improve mobility, independence, and quality of life.
Key Responsibilities:
- Evaluate patients in their home environment and develop individualized treatment plans.
- Provide therapeutic exercises, mobility training, and functional rehabilitation.
- Assess home safety and recommend appropriate mobility or assistive equipment.
- Monitor patient progress and modify treatment plans as needed.
- Educate patients and caregivers on exercises, safety, and home-care programs.
- Collaborate with physicians and the home health care team.
- Maintain accurate clinical documentation and visit records.
Qualifications:
- Graduate of an accredited Physical Therapy program.
- Current and active PT license as required by the state.
- Current BLS certification.
- Previous home health or rehabilitation experience preferred.
- Strong assessment, communication, organization, and patient-care skills.
Pay Rate
$60.47 - $65.35/hour
