Position Summary:

We are seeking a compassionate and dedicated Home Health & Hospice Registered Nurse (RN) to provide skilled nursing care to patients in their homes and hospice settings. The RN will assess patient needs, develop individualized care plans, manage symptoms, coordinate interdisciplinary care, and educate patients and families to promote comfort, independence, and quality of life.


Key Responsibilities:

  • Conduct comprehensive patient assessments and develop individualized care plans
  • Provide skilled nursing care, including medication administration, wound care, IV therapy, and symptom management
  • Monitor patient conditions and adjust care plans in collaboration with physicians and the interdisciplinary team
  • Deliver hospice and palliative care focused on pain management, comfort, and quality of life
  • Educate patients, families, and caregivers on disease management, medications, home safety, and end-of-life care
  • Coordinate care with physicians, therapists, social workers, hospice staff, and other healthcare professionals
  • Perform home visits and complete required documentation in the Electronic Medical Record (EMR)
  • Assist with admissions, recertifications, discharge planning, and care transitions
  • Respond to changes in patient condition and provide appropriate interventions
  • Ensure compliance with HIPAA, infection prevention standards, and home health and hospice regulations
  • Participate in on-call rotation and quality improvement initiatives as required

Required Qualifications:

  • Active, unrestricted Registered Nurse (RN) license in the state of practice
  • Graduate of an accredited Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) program
  • Current BLS/CPR certification required
  • Minimum 1 year of recent Home Health, Hospice, or related nursing experience preferred
  • Strong knowledge of home health, hospice, palliative care, wound care, medication management, and patient assessment
  • Excellent communication, critical-thinking, organizational, and interpersonal skills
  • Experience with Electronic Medical Record (EMR) systems preferred
  • Valid driver's license and reliable transportation for patient visits
  • Ability to work independently and effectively manage a patient caseload
