Position Summary:
We are seeking a compassionate and experienced Home Health & Hospice Registered Nurse (RN) to provide high-quality, patient-centered care to patients in their homes or hospice settings. The RN will perform comprehensive assessments, develop individualized care plans, administer treatments, provide symptom and pain management, educate patients and families, and collaborate with the interdisciplinary team to promote comfort, independence, and quality of life.
Key Responsibilities:
- Conduct comprehensive nursing assessments in home health and hospice settings
- Develop, implement, and update individualized patient care plans based on clinical needs
- Administer medications, IV therapy, wound care, injections, and other prescribed treatments
- Monitor patient conditions and promptly report changes to the physician or interdisciplinary team
- Provide pain and symptom management to maximize patient comfort and quality of life
- Educate patients and caregivers on disease management, medications, treatment plans, and end-of-life care
- Coordinate care with physicians, therapists, social workers, home health aides, and other healthcare professionals
- Perform admissions, recertifications, routine visits, and discharge planning
- Maintain accurate and timely documentation in Electronic Medical Records (EMR)
- Ensure compliance with infection prevention protocols, HIPAA regulations, Medicare guidelines, and agency policies
- Participate in on-call rotations as required and respond appropriately to urgent patient needs
- Promote compassionate, respectful, and patient-centered care while supporting patients and families throughout the continuum of care
Required Qualifications:
- Active, unrestricted Registered Nurse (RN) license in the state of practice
- Graduate of an accredited Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) program
- Current BLS/CPR certification required
- Minimum 1 year of recent Home Health, Hospice, or related nursing experience preferred
- Strong knowledge of home health practices, hospice philosophy, wound care, medication administration, and chronic disease management
- Excellent clinical assessment, communication, critical-thinking, and organizational skills
- Ability to work independently and manage multiple patient visits
- Experience with Electronic Medical Record (EMR) systems preferred
- Valid driver's license, reliable transportation, and current auto insurance as required for home visits

Pay Rate
$68.09 - $72.97/hour
