Registered Nurse – Home Health & Hospice
We are seeking a compassionate Registered Nurse to provide skilled nursing care to patients in their homes and hospice settings. The RN will assess patient needs, develop care plans, manage symptoms, and provide education and emotional support to patients and families.
Responsibilities:
- Assess, monitor, and document patient conditions in home health and hospice settings.
- Administer medications, IV therapies, wound care, and other prescribed treatments.
- Develop and implement individualized care plans.
- Manage pain and symptoms to promote patient comfort and quality of life.
- Educate patients and caregivers on disease management, medications, and end-of-life care.
- Collaborate with physicians and the interdisciplinary healthcare team to coordinate care.
Qualifications:
- Active and unrestricted Registered Nurse (RN) license.
- Graduation from an accredited nursing program.
- BLS certification required.
- Strong knowledge of home health, hospice, palliative care, and case management.
- Excellent assessment, communication, and compassionate patient care skills.
- Previous Home Health, Hospice, or Palliative Care nursing experience preferred.
Pay Rate
$68.09 - $72.97/hour
