Home Care Registered Nurse
We are seeking a compassionate and experienced Home Care Registered Nurse to provide skilled nursing care to patients in their homes. The RN will assess patient needs, develop individualized care plans, provide treatments, monitor progress, and educate patients and caregivers to support safe and effective care at home.
Responsibilities:
- Conduct comprehensive nursing assessments in the patient’s home.
- Develop and implement individualized care plans.
- Provide medications, treatments, wound care, and other skilled nursing services as ordered.
- Monitor patients’ conditions and report changes promptly.
- Educate patients and caregivers on medications, treatments, disease management, and safety.
- Coordinate care with physicians, therapists, and other healthcare professionals.
- Assist patients with managing chronic and acute health conditions.
- Maintain accurate and timely clinical documentation.
- Promote patient safety, infection control, and proper care practices.
- Respond appropriately to changes in patient condition and healthcare needs.
- Support continuity of care and help patients achieve their health goals.
Qualifications:
- Current Registered Nurse (RN) license in the applicable state.
- Nursing degree from an accredited program.
- Previous home health or home care experience preferred.
- Strong knowledge of skilled nursing care, patient assessment, and care planning.
- Excellent communication, clinical judgment, and organizational skills.
- Ability to work independently and manage multiple patient needs.
- Compassionate, dependable, and patient-focused.
Pay Rate
$68.09 - $72.97/hour
