Position Summary:
We are seeking a compassionate and experienced Home Care Registered Nurse to provide skilled nursing care to patients in their homes. The RN will assess patient needs, provide clinical interventions, administer medications and treatments, educate patients and caregivers, and coordinate care to promote safety, comfort, and independence.
Key Responsibilities:
- Perform comprehensive nursing assessments in the home setting.
- Develop, implement, and evaluate individualized care plans.
- Provide skilled nursing services based on patient needs and physician orders.
- Administer medications, injections, IV therapies, and prescribed treatments as appropriate.
- Provide wound care, catheter care, ostomy care, and other nursing interventions.
- Monitor patient conditions and identify changes requiring medical intervention.
- Educate patients and caregivers on medications, disease management, treatment plans, and home safety.
- Assist patients and families with disease management and self-care strategies.
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
- Maintain accurate and timely clinical documentation in the EMR.
- Participate in care planning, follow-up visits, and discharge planning.
- Follow infection prevention, HIPAA, patient safety, and home care policies.
Required Qualifications:
- Active, unrestricted Registered Nurse (RN) license in the state of practice.
- ADN or BSN from an accredited nursing program.
- Current BLS certification required.
- Minimum 1 year of recent home care, home health, or clinical nursing experience preferred.
- Strong knowledge of patient assessment, medication administration, wound care, and chronic disease management.
- Excellent clinical judgment, communication, organization, and time-management skills.
- Ability to work independently in the home environment.
- Valid driver's license and reliable transportation for patient visits.
Preferred Qualifications:
- BSN preferred.
- Previous home health or Medicare-certified home care experience.
- OASIS experience preferred.
- Experience caring for geriatric, post-surgical, medically complex, or chronically ill patients.
- Wound care, IV therapy, or infusion experience preferred.
- Strong patient and caregiver education skills.
- Commitment to compassionate, patient-centered care.
Pay Rate
$68.09 - $72.97/hour
