Position Summary:
We are seeking a compassionate and dedicated Home Care Physical Therapist (PT) to provide skilled physical therapy services to patients in their homes. The Physical Therapist will evaluate patients, develop individualized treatment plans, improve mobility and functional independence, and educate patients and caregivers to promote recovery and enhance quality of life.
Key Responsibilities:
- Perform comprehensive physical therapy evaluations in patients' homes.
- Develop and implement individualized treatment plans based on patient needs and physician orders.
- Provide therapeutic exercises, gait training, balance training, manual therapy, and functional mobility training.
- Evaluate patient progress and modify treatment plans as appropriate.
- Educate patients and caregivers on home exercise programs, fall prevention, safe mobility, and proper use of assistive devices.
- Recommend adaptive equipment and home safety modifications to improve independence.
- Maintain accurate documentation, progress notes, and care plans in the Electronic Medical Record (EMR).
- Coordinate care with physicians, nurses, occupational therapists, speech-language pathologists, case managers, and other healthcare professionals.
- Participate in discharge planning and ensure continuity of care.
- Comply with home health regulations, infection prevention protocols, HIPAA requirements, and agency policies.
- Participate in quality improvement initiatives and continuing education activities.
Required Qualifications:
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited Physical Therapy program.
- Active Physical Therapist (PT) license in the state of practice.
- Current BLS certification required.
- Minimum 1 year of recent home health or physical therapy experience preferred.
- Valid driver's license, reliable transportation, and current auto insurance as required.
- Strong knowledge of rehabilitation principles, patient mobility, therapeutic exercise, and home health practices.
- Excellent communication, organizational, and interpersonal skills.
- Experience with Electronic Medical Record (EMR) systems preferred.
Pay Rate
$60.47 - $65.35/hour
