Position Summary:
We are seeking a compassionate and skilled Home Care Physical Therapist to provide in-home rehabilitation services to patients recovering from illness, injury, surgery, or chronic conditions. The Physical Therapist will evaluate patients, develop individualized treatment plans, and deliver evidence-based therapy to improve mobility, independence, safety, and overall quality of life in the home setting.
Key Responsibilities:
- Perform comprehensive physical therapy evaluations in patients' homes
- Develop, implement, and modify individualized treatment plans based on patient needs and goals
- Provide therapeutic exercises, gait training, balance training, transfer training, neuromuscular re-education, manual therapy, and functional mobility interventions
- Assess home environments and recommend modifications or adaptive equipment to improve patient safety and independence
- Educate patients and caregivers on home exercise programs, fall prevention, body mechanics, pain management, and safe use of assistive devices
- Monitor patient progress and adjust treatment plans to maximize functional outcomes
- Recommend and train patients in the use of walkers, canes, wheelchairs, orthotics, prosthetics, and other mobility aids
- Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, case managers, and other healthcare professionals to coordinate patient care
- Maintain accurate and timely documentation in Electronic Medical Records (EMR)
- Participate in care planning, discharge planning, and quality improvement initiatives
- Ensure compliance with home health regulations, infection prevention protocols, HIPAA, and patient safety standards
- Travel to patient homes within the assigned service area while providing compassionate, patient-centered care
Required Qualifications:
- Active, unrestricted Physical Therapist (PT) license in the state of practice
- Doctor of Physical Therapy (DPT) degree from an accredited Physical Therapy program
- Current BLS/CPR certification required
- Minimum 1 year of recent physical therapy experience required; home health experience preferred
- Strong knowledge of rehabilitation techniques, home health practices, functional mobility, and evidence-based physical therapy
- Excellent clinical assessment, communication, documentation, and critical-thinking skills
- Experience with Electronic Medical Record (EMR) systems preferred
- Valid driver's license, reliable transportation, and current auto insurance for home visits
- Ability to work independently and effectively manage a daily caseload
