Position Summary:
We are seeking a skilled Physical Therapist (PT) to evaluate and treat patients with injuries, illnesses, or physical disabilities. The PT will develop individualized treatment plans to improve mobility, strength, balance, and functional independence while supporting patients throughout their rehabilitation.
Key Responsibilities:
- Perform comprehensive physical therapy evaluations and functional assessments.
- Develop and implement individualized treatment plans based on patient needs and goals.
- Provide therapeutic exercises, manual therapy, gait training, balance training, and mobility interventions.
- Monitor patient progress and modify treatment plans as needed.
- Educate patients and caregivers on home exercise programs, safety, and injury prevention.
- Recommend and train patients on appropriate assistive devices and adaptive equipment.
- Maintain accurate treatment notes, progress reports, and discharge documentation.
- Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other healthcare professionals.
- Participate in interdisciplinary care planning and patient conferences.
- Follow infection prevention, HIPAA, patient safety, and organizational policies.
- Participate in quality improvement and continuing education activities.
Required Qualifications:
- Doctor of Physical Therapy (DPT), Master's, or Bachelor's degree from an accredited Physical Therapy program.
- Active Physical Therapist (PT) license in the state of practice.
- Current BLS certification required.
- Minimum 1 year of recent Physical Therapy experience preferred.
- Strong knowledge of rehabilitation, mobility, therapeutic exercise, and patient care.
- Excellent communication, critical-thinking, organizational, and interpersonal skills.
- Experience with EMR systems preferred.
Preferred Qualifications:
- Experience in acute care, inpatient rehabilitation, outpatient, SNF, home health, or long-term care settings.
- Experience treating orthopedic, neurological, geriatric, post-surgical, or medically complex patients.
- Board certification or specialty certification preferred.
- Ability to work effectively within an interdisciplinary rehabilitation team.
- Commitment to compassionate, patient-centered care.
Pay Rate
$64.28 - $69.16/hour
