Licensed Practical Nurse – Long-Term Care (LTC LPN)
We are seeking a compassionate and dependable Licensed Practical Nurse to provide high-quality nursing care to residents in a long-term care setting. The LTC LPN will administer medications, monitor resident health, support daily care needs, and collaborate with the healthcare team to ensure exceptional resident outcomes.
Responsibilities:
• Provide direct nursing care to residents in a long-term care environment.
• Administer medications, treatments, and therapeutic interventions as prescribed.
• Monitor resident conditions, assess changes, and report concerns to the appropriate healthcare provider.
• Assist in developing, implementing, and evaluating individualized care plans.
• Collaborate with nurses, physicians, and interdisciplinary team members to coordinate resident care.
• Educate residents and families on health conditions, medications, and care plans.
• Maintain accurate and timely clinical documentation.
• Promote a safe, comfortable, and supportive environment for residents.
Qualifications:
• Active and unrestricted Licensed Practical Nurse (LPN) license.
• Graduation from an accredited Practical Nursing program.
• BLS certification required.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work effectively in a long-term care environment.
• Previous long-term care, skilled nursing, or rehabilitation experience preferred.
Pay Rate
$45.22 - $50.10/hour
