Position Summary:

We are seeking an experienced and compassionate Director of Social Services to lead and oversee social services operations within a healthcare setting. The Director will manage the social services team, coordinate patient support programs, ensure regulatory compliance, and collaborate with interdisciplinary teams to promote patient well-being, discharge planning, and access to community resources.


Key Responsibilities:

  • Lead and supervise the Social Services department and provide guidance to social work staff
  • Conduct or oversee psychosocial assessments and develop individualized care plans
  • Coordinate discharge planning and facilitate referrals to community resources and support services
  • Collaborate with physicians, nurses, case managers, therapists, and other healthcare professionals to ensure comprehensive patient care
  • Provide counseling, crisis intervention, advocacy, and support to patients and families
  • Ensure compliance with federal, state, and facility regulations, accreditation standards, and ethical practice guidelines
  • Develop departmental policies, procedures, and quality improvement initiatives
  • Monitor department performance, staffing, and workflow to ensure efficient service delivery
  • Maintain accurate documentation in the Electronic Medical Record (EMR)
  • Participate in interdisciplinary meetings, care conferences, and leadership committees
  • Support staff education, mentoring, and professional development

Required Qualifications:

  • Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program
  • Active Social Work license (LBSW, LMSW, LCSW, or equivalent) as required by the state
  • Minimum 3 years of recent healthcare social work experience
  • Previous leadership or supervisory experience preferred
  • Strong knowledge of discharge planning, case management, patient advocacy, and community resources
  • Excellent leadership, communication, organizational, and interpersonal skills
  • Experience with Electronic Medical Record (EMR) systems preferred

Pay Rate
$79.52 - $84.40/hour
