Ambulatory Clinical Pharmacy Specialist
We are seeking an experienced Ambulatory Clinical Pharmacy Specialist to provide comprehensive medication management and clinical pharmacy services in an outpatient setting, supporting safe and effective patient care.
Key Responsibilities:
- Review and optimize patient medication regimens.
- Provide medication management and patient counseling.
- Monitor medication effectiveness, safety, and adherence.
- Support chronic disease management and clinical pharmacy programs.
- Collaborate with physicians and healthcare teams on treatment plans.
- Document clinical interventions and patient outcomes.
Qualifications:
- Doctor of Pharmacy (PharmD) or equivalent pharmacy degree.
- Active Pharmacist license as required by state regulations.
- Ambulatory care or clinical pharmacy experience preferred.
- BCACP or other relevant board certification preferred.
- Strong knowledge of pharmacotherapy and medication management.
Pay Rate
$136,000.00 - $200,000.00/year
