Position Summary:

We are seeking an experienced RN Outpatient Case Manager to coordinate comprehensive care for patients in an outpatient setting. The RN will assess patient needs, develop care plans, coordinate services, monitor outcomes, provide education, and collaborate with patients, families, providers, and multidisciplinary teams.

Key Responsibilities:

  • Perform comprehensive assessments of patients' medical, functional, psychosocial, and care coordination needs.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
  • Coordinate appointments, referrals, diagnostic services, treatments, and community resources.
  • Monitor patient progress, treatment adherence, and clinical outcomes.
  • Identify barriers to care and develop strategies to improve access and continuity of care.
  • Coordinate transitions between outpatient, inpatient, specialty, and community-based services.
  • Educate patients and families regarding medications, treatment plans, disease management, and self-care.
  • Communicate with physicians, specialists, insurance providers, and other healthcare professionals as appropriate.
  • Assist with prior authorizations, referrals, and utilization management processes when required.
  • Maintain accurate and timely documentation in the EMR.
  • Participate in multidisciplinary care conferences and quality improvement activities.
  • Follow HIPAA, patient safety, infection prevention, and organizational policies.

Required Qualifications:

  • Active, unrestricted Registered Nurse (RN) license in the state of practice.
  • ADN or BSN from an accredited nursing program.
  • Current BLS certification required.
  • Minimum 1 year of recent outpatient, case management, or care coordination experience preferred.
  • Strong knowledge of care coordination, chronic disease management, and patient education.
  • Excellent clinical judgment, communication, organization, and problem-solving skills.
  • Ability to work independently and collaboratively with multidisciplinary teams.
  • Experience with EMR systems preferred.

Preferred Qualifications:

  • BSN preferred.
  • Previous outpatient clinic, ambulatory care, managed care, or population health experience.
  • Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred.
  • Experience with referrals, prior authorizations, utilization management, and insurance processes.
  • Strong knowledge of community resources and healthcare navigation.
  • Experience caring for patients with complex or chronic conditions.
  • Strong patient advocacy and care coordination skills.

Pay Rate
$61.23 - $66.11/hour
