Clinical (Hospital) Pharmacist
Position Summary
We are seeking a Clinical Pharmacist to provide comprehensive pharmaceutical care in a hospital setting, ensuring safe, effective, and evidence-based medication use for patients.
Responsibilities
- Review and verify medication orders for accuracy, appropriateness, and safety.
- Provide clinical recommendations on drug therapy, dosing, and monitoring.
- Monitor patient responses and manage medication-related problems.
- Educate patients, families, and healthcare staff on medications.
- Collaborate with the interdisciplinary team on patient care plans.
- Document interventions and maintain accurate records.
- Follow hospital policies, safety protocols, and professional standards.
Qualifications
- Doctor of Pharmacy (Pharm.D.) degree from an accredited program.
- Current pharmacist license in good standing.
- Hospital or clinical pharmacy experience preferred.
- Strong clinical knowledge, communication, and critical-thinking skills.
- Detail-oriented and patient-centered approach.
Pay Rate
$90.96 - $95.83/hour
