Clinical (Hospital) Pharmacist
We are seeking a skilled Clinical Hospital Pharmacist to provide safe and effective medication management and clinical pharmacy services to hospitalized patients.
Key Responsibilities:
Review and verify medication orders for accuracy, appropriateness, interactions, and safety.
Monitor patient medication therapy and recommend adjustments based on clinical needs.
Collaborate with physicians, nurses, and other healthcare professionals to optimize medication therapy.
Provide medication counseling and education to patients and healthcare staff.
Monitor for adverse drug reactions, allergies, and medication-related complications.
Maintain accurate medication records and follow pharmacy, hospital, and safety standards.
Qualifications:
Doctor of Pharmacy (PharmD) or equivalent pharmacy degree.
Current Pharmacist license in the applicable state.
Hospital or clinical pharmacy experience preferred.
BLS certification preferred.
Strong clinical knowledge, attention to detail, communication, and medication management skills.
Pay Rate
$90.96 - $95.83/hour
