Job Summary
The RN Care Manager – Emergency Department (ED) is responsible for coordinating patient care and transitions across the continuum, with a focus on safe, timely, and appropriate disposition from the Emergency Department. This role collaborates with ED clinicians, inpatient teams, social work, and community providers to ensure effective care coordination, utilization management, and discharge planning while maintaining compliance with regulatory and quality standards.
Essential Duties & Responsibilities

Conduct comprehensive clinical assessments of ED patients to determine care needs, level of care, and appropriate disposition

Coordinate care with emergency physicians, nurses, social workers, inpatient units, and ancillary services

Perform utilization review to ensure medical necessity and appropriate use of hospital resources

Facilitate timely discharge planning directly from the ED, including referrals to home care, skilled nursing facilities, rehabilitation, behavioral health services, and community resources

Complete PRI and Screen assessments as required

Identify high-risk patients and implement interventions to reduce avoidable admissions and ED revisits

Educate patients and families regarding care plans, discharge instructions, and follow-up services

Advocate for patients to ensure access to appropriate services and continuity of care

Maintain accurate, timely, and compliant documentation in the electronic health record

Ensure compliance with hospital policies, regulatory requirements, and evidence-based practice standards

Participate in interdisciplinary rounds, quality improvement initiatives, and performance improvement activities

Provide clinical guidance and support to ED staff related to care management processes
Required Skills & Experience

Minimum of five (5) years of clinical experience as a Registered Nurse
-
Experience with care coordination, utilization review, and discharge planning

Strong clinical assessment, critical thinking, and decision-making skills
-
Ability to function effectively in a fast-paced Emergency Department environment

Computer literacy, including proficiency with electronic health record (EHR) systems

Excellent communication, organizational, and interpersonal skills

Leadership experience

Prior experience in emergency department, care management, or case management roles
Required Education
-
Associate’s Degree in Nursing (ASN or ADN) from an accredited nursing program
Preferred Education

Bachelor’s or Master’s Degree in Nursing
Required Certifications & Licensure
-
Active Registered Nurse (RN) license in the State of New York
-
PRI and Screen certification within three (3) months of employment
-
Primary Source Verification of license and registration as a Registered Professional Nurse from the New York State Department of Education (NYSED)

Basic Life Support (BLS) certification
Preferred Certifications & Licensure

Case Management Certification (CCM or equivalent)
Work Environment & Expectations

Based in a high-volume Emergency Department setting

Requires collaboration with interdisciplinary teams and external agencies

Involves frequent standing, walking, and patient interaction

May require flexibility in scheduling, including weekends or holidays

Adherence to HIPAA, patient safety standards, and professional nursing ethics is required
Pay Rate
$65.58 - $70.46/hour
