Registered Nurse (RN) – Inpatient Care Coordinator
We are seeking a skilled Inpatient Care Coordinator RN to coordinate patient care, support discharge planning, and ensure effective communication across the healthcare team.
Responsibilities:
Assess patient needs and coordinate individualized care plans.
Coordinate care with physicians, nurses, and interdisciplinary teams.
Assist with discharge planning and transitions of care.
Monitor patient progress and identify barriers to care.
Coordinate referrals, follow-ups, and healthcare services.
Educate patients and families on care plans and available resources.
Maintain accurate patient records and documentation.
Promote quality, safety, and continuity of patient care.
Qualifications:
Graduate of an accredited Nursing program.
Active Registered Nurse (RN) license as required by state regulations.
Current BLS certification required.
Strong knowledge of care coordination, discharge planning, and inpatient nursing.
Excellent communication, organization, and critical-thinking skills.
Previous inpatient, case management, or care coordination experience preferred.
Pay Rate
$81.81 - $86.69/hour
