Registered Nurse (RN) – Case Manager

We are seeking an experienced Registered Nurse (RN) Case Manager to coordinate patient care, support appropriate healthcare services, and help achieve quality patient outcomes.

Key Responsibilities:

Assess patient needs and develop individualized care plans.
Coordinate care and services across healthcare providers.
Monitor patient progress and facilitate transitions of care.
Identify barriers to care and connect patients with appropriate resources.
Collaborate with physicians and interdisciplinary teams.
Maintain accurate case management documentation.

Qualifications:

Graduate of an accredited Nursing program.
Active RN license as required by state regulations.
Current BLS certification preferred/required.
Previous Case Management, Care Coordination, or acute care experience preferred.
Strong clinical assessment, communication, and organizational skills.

Pay Rate
$83.33 - $88.21/hour
