Position Summary:
We are seeking an experienced Registered Nurse Case Manager to coordinate comprehensive, patient-centered care across the healthcare continuum. The RN Case Manager will assess patient needs, develop individualized care plans, coordinate services, monitor outcomes, and collaborate with patients, families, providers, and multidisciplinary teams.
Key Responsibilities:
- Perform comprehensive assessments of patients' medical, functional, psychosocial, and care coordination needs.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
- Coordinate services across physicians, specialists, hospitals, rehabilitation, home health, and community resources.
- Monitor patient progress, treatment plans, and outcomes.
- Identify barriers to care and develop strategies to support timely and appropriate services.
- Facilitate transitions of care, admissions, discharges, and follow-up services.
- Educate patients and families regarding treatment plans, medications, available resources, and self-management.
- Communicate with insurance providers and other stakeholders regarding authorizations and care needs as applicable.
- Maintain accurate and timely documentation in the EMR.
- Identify opportunities to improve quality, patient outcomes, and cost-effective care.
- Participate in multidisciplinary case conferences and quality improvement activities.
- Follow HIPAA, patient safety, infection prevention, and organizational policies.
Required Qualifications:
- Active, unrestricted Registered Nurse (RN) license in the state of practice.
- ADN or BSN from an accredited nursing program.
- Current BLS certification required.
- Minimum 1 year of recent case management, care coordination, or clinical nursing experience preferred.
- Strong knowledge of care coordination, discharge planning, utilization management, and community resources.
- Excellent communication, organization, critical-thinking, and problem-solving skills.
- Ability to work independently and collaboratively with multidisciplinary teams.
- Experience with EMR systems preferred.
Preferred Qualifications:
- BSN preferred.
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred.
- Previous experience in acute care, managed care, home health, rehabilitation, or outpatient case management.
- Knowledge of insurance authorization and utilization review processes.
- Strong patient advocacy and care coordination skills.
- Commitment to compassionate, patient-centered care.
Pay Rate
$83.33 - $88.21/hour
