Position Summary:
We are seeking a compassionate and experienced Registered Nurse (RN) to provide comprehensive nursing care in an Acute Care Center / Skilled Nursing setting. The RN will assess patient conditions, administer treatments, coordinate care, and collaborate with the interdisciplinary team to deliver high-quality care for patients requiring short-term rehabilitation, post-acute recovery, and skilled nursing services.
Key Responsibilities:
- Perform comprehensive patient assessments and develop individualized nursing care plans
- Provide direct nursing care to patients requiring skilled nursing and post-acute services
- Administer medications, IV therapies, wound care, tube feedings, and other prescribed treatments
- Monitor patient conditions, vital signs, laboratory results, and responses to treatment
- Coordinate admissions, transfers, and discharges
- Educate patients and families regarding medications, treatments, rehabilitation goals, and discharge planning
- Collaborate with physicians, therapists, case managers, social workers, and other healthcare professionals
- Supervise and delegate appropriate patient care activities to LPNs and CNAs
- Maintain accurate documentation in the Electronic Medical Record (EMR)
- Ensure compliance with infection prevention protocols, HIPAA regulations, patient safety standards, and facility policies
- Participate in interdisciplinary care conferences, quality improvement initiatives, and continuing education activities
Required Qualifications:
- Active, unrestricted Registered Nurse (RN) license in the state of practice
- Graduate of an accredited Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) program
- Current BLS certification required
- Minimum 1 year of recent experience in Skilled Nursing, Long-Term Care, Rehabilitation, Post-Acute Care, or Medical-Surgical nursing preferred
- Strong knowledge of medication administration, wound care, chronic disease management, rehabilitation nursing, and patient assessment
- Excellent communication, critical-thinking, organizational, and interpersonal skills
- Experience with Electronic Medical Record (EMR) systems preferred
Preferred Qualifications:
- BSN preferred
- Experience in skilled nursing facilities (SNF), rehabilitation centers, transitional care units, or acute care settings
- IV Therapy certification preferred
- Previous travel or contract nursing experience preferred
- Wound Care Certification (WCC) or Gerontological Nursing Certification (RN-BC) preferred
- Strong teamwork, adaptability, and commitment to patient-centered care
Pay Rate
$56.66 - $61.53/hour
